Service Provider Application - Home Page

Introduction

The Home Page is the starting point for all Service Provider Central activities. On this page, service providers can log into their account and update their organization's data, create a new account, reset their password, or change their password.


Logging Into An Existing Account

To log into an existing service provider account, the service provider's primary or alternate contact must go to the Home Page and enter the email address that NDVH has on file for them in the email field and their password in the password field, and then click the Login button.


Creating A New Account

Service providers not currently in the NDVH database can create a new account and submit their organization's information to NDVH to be included in the database. To create a new account, the person who will maintain the organization's information should follow these steps:

  1. Go to the Home Page.
  2. Click the Create new account hyperlink, which will open the Check For Existing Account form.
  3. Enter the contact email address and click the Submit button to check for an existing account.
  4. If an account does not already exist with that email address, a message will appear saying so. Click the Continue button, which will open the Primary Contact Information form.
  5. Complete the form, and then click the Continue button. This will open the Provider Detail page.
  6. Complete the Provider Detail page, and then click the Save & Submit button to save the data to the database and submit it to NDVH for review.


Changing Your Password

A service provider's primary or alternate contact can change the password that is on file for them in the NDVH database by following these steps:

  1. Go to the Home Page.
  2. Click the Change password hyperlink, which will open the Change Your Password form.
  3. Enter the required information in the Change Your Password form and click the Change button.
  4. If the password is changed successfully, a message will appear saying so.


Resetting Your Password

A service provider's primary or alternate contact can reset the password that is on file for them in the NDVH database by following these steps:

  1. Go to the Home Page.
  2. Click the Forgot password? hyperlink, which will open the Password Recovery form.
  3. Enter the email address that is on file for them in the NDVH database in the email field, and then click the Submit button.
  4. If the password is reset successfully, a message will appear saying so, and an email will be sent to the email address with the new account information.
