Service Provider Application - Provider Information Page

Introduction

The Provider Information page shows the detailed information for a service provider in the NDVH database. That information includes organization description, location city/state/county and mailing address, primary and alternate contact person, organization phone number(s), states and counties served, and services provided.


Editing A Service Provider

To edit a existing service provider, enter the provider information in the fields provided. Click the Save button to save changes to the database. When finished, click the Save & Submit button to save changes to the database and to submit the data to NDVH for review. An NDVH administrator will review the submission for completness and import the data into the hotline database.


Marking A Service Provider As Up-To-Date

Periodically, NDVH conducts service provider data update campaigns in which the hotline contacts the service providers in its database and requests that they update their organzation's information in the hotline's database. This helps to ensure that the hotline can provide its callers with the most accurate and appropriate referrals for crisis assistance and intervention.

If you received correspondence from NDVH requesting that you update your organization's information in the NDVH database and your organization's information has not changed (meaning that the data currently in the NDVH database is completely accurate), you can mark your organization's information as up-to-date by clicking the No Changes button. This will submit the information as-is to the hotline mark it up-to-date.
